Please complete a separate registration form for each person attending the Congress. If payment is required by check, please contact the Registrar at 314-645-1455. Congress registration cancellations must be submitted in writing via email to the Registrar no later than October 25, 2013 for a full refund, less a $75.00 processing fee. Cancellations received after October 25th will be refunded 50% of the registration fee. Cancellations received on or after November 1st will not receive a refund. Substitute attendees are permitted, please give advance notice if possible.

Sleeping Room Information:

Sleeping room reservations will be accepted until Monday, October 7, 2013. Reservations made after this date cannot be guaranteed at the RFC conference rate of $109.00 per night, plus tax, for a queen single or double room. Only one reservation per room is needed. If you are sharing a room please coordinate with your roommate before completing this form.
